Primary Hyperparathyroidism



Abstract
The paper presents a case of Primary Hyperparathyroidism duo to parathyorid adenoma operated upon at the St. Paul¢¥s Hospital.
A 36 year-old woman was admitted to, the Hospital in a bed-ridden state of immobility because of severe bone pain and generalized muscular weakness: The patient had a long history of illness with gradual deterioration of the above symptoms for preceding 5 years.
At the time of admission, multiple spontaneous fractures on ribs scapula, clavicle and iliac bones were noted; but no signs of urinary and G-I system were found.
Pre-operatively, repeated serum calcium level showed 10. 0-12.4m0/6 and phosphorous 1.8mg%.
Exploration of the neck disclosed a tumor mass located about 1.5cm below the lower pole of the right thyroid gland, with a vascular pedicle branching off from the right inferior thyr-oid artery. The mass was about 1. 5 X 1.5 X 1.0cm in size, about 900mg in weight, of dark brown reddish color, with smooth surface.
Histopathological findings of this tumor mass revealed parathyroid adenoma composed of chief cell, oxyphils and a few water-clear cells. The postoperative course was uneventful except temporary hypocalcemic condition which was readily controlled with the administration of calcium carbonate and Vit. D3 per os.
The patient was discharged from the Hospital in an excellent condition as she walked out by herself without any support on the 36 postoperative day.
